  2. Price Adjustment (eg. $5.00)
  3. Receipt Copy/Proof (attached)
  4. Your phone # that you registered with (eg. (650) 123-4567)

Please remember: submitting adjustment request form is not guarantee to have it paid. Our Drivers team will review and follow up with you, if needed.
